• Child enrollment and Health Information Form

• Child's Medical Statement (for preschool and toddlers only)

2. Complete forms

3. Call 792-0905 for an appointment to bring forms in. Completed forms with registration fee secures your space. Limited spaces available, enrollment is on first come first serve basis.
